Using the CRIC-ALOT

Yes. CRIC-ALOT is an educational task trainer and is not for use on patients.

Each kit includes a male and a female larynx model, 8 replacement tracheas (4 male and 4 female), 200 skin pouches, and 200 cotton pads, all packed in a briefcase-style nylon corrugated carrying case.

The male model has a prominent thyroid cartilage (Adam’s apple). On the female model, which is about 15% smaller, the thyroid and cricoid are about equally prominent.

Put a 4×4" cotton pad inside a skin pouch. Add more layers to simulate a patient with obesity.

Optional: add 10–20 cc of fake blood for a wet, bleeding model.  Peel back the pouch backing. Carefully place the adhesive side on the model, starting with the edge marked TOP at the hyoid, then press the rest smoothly over the larynx and trachea. Adjust the sides so the larynx can still move laterally, from side to side. This lets you practice the laryngeal handshake.  Setup takes [under a minute] per rep.

Hold the bottom aspect of the case in your left hand and grab the sides of the case (with vertical grooves) with your right hand.  Gently compress the grooves on either side with your right hand and slide the case open.  Slide the top of the case down the ramp until it locks. It then acts as a sternum, so you can practice stabilizing your scalpel hand. The top also detaches, which lets you show the anatomy of the larynx and trachea.  Removing the top (by compressing the sides more) permits visual demonstration of anatomy and common procedural errors. To demonstrate over-insertion of a 6.0 tracheal tube, pass the tube into the CTM until the pilot balloon; note that the tip projects beyond the trachea length (where the bifurcation of right and left mainstem occurs).  To demonstrate hyo-thyroid misplacement pass a 6-0 tube between the hyoid and thyroid and insert to the pilot balloon; esophageal placement (below the trachea) is evident when the top of the model case is removed.

Use whatever devices you use for cricothyrotomy in your clinical practice, i.e., scalpel, tracheal tube, bougie, or dedicated Cric kits (such as the Control Cric by Intersurgical, or the Chinook Universal Stylet Bougie Assisted Cricothyrotomy (USBCR).

We recommend size 6.0mm ID tracheal tubes if you use a standard tracheal tube; most of these sized tubes have an outer diameter of 8.2mm, which fits into the cricothyroid membrane space of adult patients (and the Cricalot models).

The Universal Stylet Bougie is particularly useful for bougie aided cric because it can be shaped with a distal J hook to verify tracheal ring palpation on insertion; it also permits preloading the tube.  Standard bougies do not permit either tracheal ring feedback or easy preloading.

Plan on ~25-50 reps to master the basic steps, then about 10-20 reps per quarter to keep up muscle memory. With 200 skins, one kit supports a full training cycle for [a small team].

Yes. Seal it inside the pouch before you apply the skin. You can also seal the pouch (by folding the top edge onto itself) to overfill it.  When the operator cuts the top layer of the pouch blood will come out.  Afterward, rinse the model with water. Some fake blood formulations may stain the white plastic components of the CRIC-ALOT.

The male and female larynx models and the case are reusable. With normal use, the models last for thousands of reps.  There may be small nicks from scalpels on the plastic cartilages of the models.  These can be sanded down with a fine sandpaper if desired.

Skin pouches: one per rep.

Cotton pads: one or more per rep (using multiple cotton pads simulates obesity).

Tracheas rarely need replacing; they are cut only if the operator performs the procedure incorrectly.  Each male and each female model comes with one trachea preloaded, two in the blue CRIC-ALOT box, and two extras in the briefcase (5 total).

Pennies. Skin refill packs come in [400 for $50 and 1,000 for $100], so each skin pouch with a cotton pad costs about [5–12¢]. The pads are standard 4×4" non-woven cotton wipes, costing under 5¢ each from Amazon, Walmart, or most pharmacies.

Because cotton pads are widely available, or users can use cast padding, or other items from the hospital, we do not sell the padding used for the subcutaneous layer of the "skins". We recommend 4×4" non-woven cotton pads for ease of insertion, or cotton cast padding.  Non-woven materials work best (as opposed to standard 4x4 gauze pads), and cotton works better than synthetic materials (holds fake blood better, separates easier, and does not dull scalpel blades as quickly).

Maintenance & Repair

Peel off the used skin pouch. Remove any residual adhesive.  If adhesive material is not easily peeled off use Goo Gone or a similar adhesive remover.  Rinse off fake blood with water. The case is washable too.

When the procedure is performed correctly, the user cuts only the top layer of the skin pouch (top plastic layer only), then changes the scalpel's orientation and cuts the cricothyroid membrane horizontally (through the cotton pad and the back adhesive layer of the pouch). The trachea should not be incised.  If the incision location is incorrect or the user accidentally cuts aggressively with the initial vertical incision, the trachea can be damaged.  If the incision is large and affects the model's use, replace the trachea (see below).

Replace the trachea if it has been cut in a way that affects the model's use (i.e., split longitudinally, or with a large horizontal incision). First identify the male vs. the female trachea; the male trachea is larger and slightly longer than the female.   Male and female tracheas are different sizes, so use the matching one. They are not interchangeable. Each trachea has small bumps at the distal lower end.  Slide the trachea into the grooves along each side of the white tracheal ramp attached to the model; ensure the ends without the small bumps are inserted first.

It is very uncommon to need more tracheas than the kit supplies (5 for each male and female model).  Please contact us if you need more.  We sell them in packs of 10 (5 female, 5 male) for $25, plus shipping.
